Leslie is living with dementia and has also been diagnosed with an aggressive cancer. We deliver a home care package of 84 hours per week to support him and his wife who had previously been caring for him herself.
Four times a day, Leslie will be visited by two carers and his wife provides support in between. This package also involves working with an external agency and the Duty Lead Nurse from the CCG which requires great communication to ensure that everyone is working in the same way. A joint risk assessment was completed at the start and assessments were shared to help with consistency.
Both Leslie and his wife have built great relationships with his carers, and Leslie’s wife is now able to take some time for herself and enjoy doing things that she couldn’t while caring for Leslie on her own.
