Supporting safe reintegration into society

We deliver specialist forensic transitional support to adults and young people of all genders as they move from prison, youth custody, or secure hospital settings into the community. Working nationally with HMPPS, NHS England, and local authorities, we enable individuals to reintegrate safely, purposefully, and with dignity—supported every step of the way.

Our care is provided within the individual’s own home or in supported accommodation arranged by the local authority or partners. Our integrated care model ensures each person receives the structure, safety, and consistency they need—wherever they live.

We begin working with individuals while they are still in secure environments, often over a 3–6 month transition period, helping them build trust, resilience, and readiness for the community.

Case Study: Client N

Client N had spent over 12 months in seclusion in a secure hospital. Due to his learning disability and high-risk profile, he was placed in isolation to protect him from others and from serious self-harm. Over time, he became non-verbal, unkempt, and fearful, showing signs of rapid deterioration. Newcross were invited to assess his case. A dedicated team of six carers began working on the ward, taking time to build trust and learn his triggers, management plan, and safe de-escalation techniques. With time and consistency, N stabilised and was eventually supported to transition back to his family home. Today, Client N is thriving. He’s speaking confidently, enjoying football, and recently got his first tattoo. From total isolation to meaningful connection, N is now part of his community again—with the support and care he needed to get there. At Newcross Community Care, we believe that everyone—regardless of past circumstances—deserves a chance to recover, to belong, and to live safely and meaningfully in their community. Our forensic transitional care services are built on compassion, structure, and deep clinical experience.

Frequently Asked Questions

Here, you'll find guidance on care plans, support options, and more. If you have any questions, we're always happy to assist and guide you through the process.

What is your coverage area and response time for new referrals?

We provide services nationally across the UK, working with HMPPS, NHS England, and local authorities in all regions. Our dedicated referral coordinator responds to initial enquiries within 24 hours and can mobilise support very quickly - often within a matter of days when urgent placements are needed. We also support planned transitions over 3-6 months, beginning engagement while individuals are still in secure settings to ensure proper preparation and relationship building for successful community reintegration.

Do you provide accommodation or just the care support?

We provide the specialist care and support services, but we don't own or directly provide accommodation. We work collaboratively with local authorities, housing providers, and commissioners to deliver our integrated care model in accommodation you arrange - whether that's the individual's own home or supported accommodation.

What clinical qualifications do your staff have for managing high-risk individuals?

Our teams include Registered Mental Health Nurses (RMNs) and Registered Learning Disability Nurses (RNLDs) with extensive experience in both inpatient and community forensic settings. All clinicians are trauma-informed and skilled in risk management, safeguarding, and supporting complex presentations including offending histories and high-risk needs.

How do you manage individuals under MAPPA or other risk management frameworks?

We work directly with probation and justice partners as part of our collaborative MDT approach. Our robust risk management strategies are designed to support individuals under MAPPA, Jigsaw, or other frameworks, with 24-hour care available where required and strong safeguarding protocols throughout.

What settings can you provide support in?

We deliver 1:1 commissioned care across hospital settings, Mother and Baby Units, residential facilities, and in the community including the family home. We provide continuity of care during transitions between settings.
