Job Title:Community Care Lead

Reporting to:Community Care Registered Manager

Location:Hybrid – Somerset, Taunton (must be able to cover areas in Somerset and North Devon when required)

Salary:£25,000 + up to £5,000 annual bonus + car allowance (£320 per month)

Hours:Monday – Friday, 09:00 – 17:30 (37.5 hours per week)

Some flexibility will be required to include working some evenings and 1 in 4 weekends.

Who are you?

Your primary mission as a Community Care Lead at Newcross Healthcare is to deliver high-quality, person-centred care to individuals within their own homes or in the community. You will play a key role in ensuring clients receive the care and support they need to maintain independence and wellbeing. Working closely with clients and families, you will develop and implement tailored care plans that may include assistance with daily living, medication administration, mobility support, and emotional companionship.

What will you do?

Whilst a full job description can be supplied, here are a few key responsibilities for the role:

  • Take full day-to-day responsibility for the service delivery of the care package. Conduct periodic assessments and evaluations to ensure care plans are up to date and reflect clients’ evolving needs.
  • Build positive relationships with service users and their families, advocating for their best interests and ensuring they receive appropriate care and support.
  • Accurately record data into systems (e.g. bookings, complaints, incident management) and maintain clear, up-to-date records and reports.
  • Provide information, education, and support to families and caregivers to help them understand and participate in the care process.
  • Write care plans for non-clinical or non-complex clients (e.g. personal care or enabling packages) and collaborate with the Lead Nurse for implementation.
  • Undertake full assessments of new clients for non-complex packages, with support and sign-off by the Lead Nurse.
  • Be part of the regional on-call rota, providing out-of-hours support and assistance as needed.
  • Identify and report environmental or personal risks in the home environment for staff and service users.
  • Take an active role in recruitment, induction, and allocation of new staff, ensuring processes are robust and introductions to clients are well managed.
