Job Title:Community Care Lead (Fixed Term Contract – up to 12 months maternity cover)

Reporting to:Community Care Registered Manager

Location:Hybrid – must be able to travel to Perth, Edinburgh and Glasgow when required

Salary:£27,500 + up to £3,000 annual bonus + car allowance (£320 per month)

Hours:Monday to Friday, 9:00am – 5:30pm (37.5 hours per week). Some flexibility required, including occasional evenings and 1 in 4 weekends.

Who are you?

As aCommunity Care Leadat Newcross Healthcare, you will deliver high-quality, person-centred care to individuals within their homes and local communities. You will ensure service users receive the support they need to maintain independence and wellbeing, while working closely with families and colleagues to implement tailored care plans. Through your compassionate and professional approach, you’ll enhance quality of life for those you support while upholding the values of Newcross Healthcare.

What will you do?

Whilst a full job description can be supplied, here are some of the key responsibilities:

  • Take day-to-day responsibility for service delivery of assigned care packages, conducting regular reviews and ensuring care plans remain current and person-centred.
  • Build positive relationships with service users and families, advocating for their needs and ensuring high-quality care and support.
  • Maintain accurate and up-to-date records of all care activities, incidents, and assessments using relevant systems.
  • Provide education and guidance to families and caregivers, helping them participate in care delivery.
  • Write and implement non-clinical care plans (e.g. personal care and enabling packages) and collaborate with the Lead Nurse on clinical elements.
  • Undertake assessments for new non-complex clients, with support and sign-off from the Lead Nurse.
  • Participate in the on-call rota (1 in 4 weekends) to provide out-of-hours support and advice.
  • Identify and report environmental or personal risks in client homes, ensuring safety for both staff and service users.
  • Play an active role in the recruitment, induction, and allocation of new staff, ensuring thorough onboarding and introductions to clients.
