Community Care Lead (Fixed term contract, up to 12 months maternity cover)

Community Care Registered Manager
Location:
Hybrid. You must be able to travel to Perth, Edinburgh, and Glasgow when required.
Salary:
£27,500 plus up to £3,000 annual bonus, plus car allowance (£320 per month)
Hours:
Monday to Friday, 9:00am to 5:30pm (37.5 hours per week). Some flexibility is required, including occasional evenings and one in four weekends.

Who are you?
As a Community Care Lead at Newcross Healthcare, you will deliver high quality, person-centred care to individuals in their homes and local communities.
You will ensure service users receive the support they need to maintain independence and wellbeing. You will work closely with families and colleagues to implement tailored care plans.

What will you do?
A full job description is available. Key responsibilities include:
- Take day to day responsibility for service delivery across assigned care packages.
- Review care regularly and ensure care plans remain current and person-centred.
- Build strong relationships with service users and families and advocate for their needs.
- Maintain accurate and up to date records of care activity, incidents, and assessments.
- Provide guidance and education to families and carers.
- Write and implement non-clinical care plans and work with the Lead Nurse on clinical elements.
- Complete assessments for new non-complex clients with Lead Nurse sign off.
- Join the on-call rota, one in four weekends, to provide out of hours support.
- Identify and report risks in client homes to ensure safety.
- Support recruitment, induction, and allocation of new staff.
